Biosecurity is unusual among fields because we cannot run experiments. We cannot deliberately release pathogens to test our defenses. We cannot conduct randomized controlled trials of pandemic preparedness strategies.

The cases documented in this chapter represent our collective laboratory - the incidents from which we must extract lessons if we are to improve. Some are catastrophic failures. Others are remarkable successes. All contain insights relevant to current challenges.

This chapter is organized into failures and successes, but the reality is messier. Most cases contain elements of both. The response to Amerithrax was in many ways a failure - it took seven years and $100 million - but it also pioneered forensic techniques that remain foundational today. Smallpox eradication was a triumph, but it occurred against a backdrop of ongoing bioweapons programs that used the same virus.

The goal is not to assign blame or celebrate victories, but to understand what happened, why, and what we can learn.

What happened: In 2014, several high-profile biosafety incidents occurred at US government laboratories:

CDC Anthrax Incident (June 2014): Researchers at CDC’s Bioterrorism Rapid Response and Advanced Technology (BRRAT) laboratory failed to fully inactivate anthrax samples before transferring them to lower-containment laboratories. Up to 84 workers were potentially exposed.
